Creating FOV Forms

• Document Information: Includes the unique FOV form number, the date of creation, and any relevant reference numbers.
• Customer Details: Essential information such as the customer's name, mobile number, address, and their GSTIN (optional) for tax compliance.
• Goods Declaration: A clear description of the goods, their quantity, and the total declared value for freight and insurance purposes.
• Vehicle & Shipment Information: Details about the truck number, driver's name, the associated LR (Lorry Receipt) number, and the shipment route.
• FOV Charges: Specify the percentage or flat rate to be applied, which is then calculated based on the declared value of the goods.
• Signature & Notes: A section for company representative and customer declaration/approval, often including fields for digital signatures and any additional important notes.

Many values are auto-calculated where applicable, streamlining data entry. Additionally, digital templates are utilized to ensure consistency and reinforce your company's branding across all generated forms.
